FINANCIAL
Insurance billing and payment arrangements for dental procedures will be handled by your dentist’s office. Charges for your anesthesia services will be separately processed by Apex. Apex is an “in network” provider for selected dental insurance companies. Apex offers CareCredit and accepts VISA, MasterCard, Discover, and American Express.
SELF-PAY, PRIVATE INSURANCE
The estimated anesthesia fee is based on the estimated procedure time, plus 30-45 minutes to allow time before and after the procedure. Depending on how much time the procedure actually takes, the actual charge may be greater or less than the estimate. We require a deposit be made to confirm your anesthesia appointment. Payment of the balance of the fee is due on the day of service. Please note that the anesthesia fee is separate from your dentist’s fee.
Because we may, or may not, be contracted with your specific insurance company, it is our office policy to do a written financial arrangement which estimates your insurance benefit and your likely portion. After you have paid your pre-arranged fee and have completed treatment, we will bill your dental or medical insurance, if any, to collect the remainder of the fees due. We will promptly refund to you any credit balance that occurs if insurance pays more than their estimated portion.
Please understand that we file insurance as a courtesy to our patients. We are not responsible for how your insurance company handles its claims or for what benefits they pay on a claim. We at no time can guarantee what your insurance will or will not do with each claim. You remain responsible for any balance that remains after insurance has paid.
MEDICAID
Apple Health patients age eight and under are always welcome. Apple Health patients over the age of eight but under age 21 will need to sign an Apple Health “Waiver” and often have a co-pay. Apex staff can assist you with the details.. Apple Health currently does not pay for dental related anesthesia services for patients over age 20. Patients in this situation are personally responsible for anesthesia fees.
